List two people other than the parents for emergencies and who authorized to pick-up your child. 

The adults listed below are authorized to pick up my child from Edutainment Learning Center. All authorized adults will be required to show identification and sign your child out. List two adults who are authorized to pick-up your child(ren). Do not include parents/guardians listed above.

Select the weeks camper will be attending ELC's Fort Washington location. You agree to be financially responsible for payment of the weeks you select whether the camper attends or not. A minimum of two weeks must be selected. All fees are non-refundable and non transferable, including lunch and field trip fees. Required
